There are days when only a cold creamy drink will do, and this Keto Salted Caramel Milkshake always hits that spot. It is thick, frosty, and tastes like a traditional milkshake without the sugar overload. It’s sweet, creamy and totally indulgent but still fits perfectly into your low carb goals.
What really makes this recipe special is the homemade salted caramel sauce blended right in. That caramel flavor runs through every sip instead of just sitting on top. It is quick, fun to make, and perfect for a last minute cool and refreshing treat.

🔪 How to Make Keto Salted Caramel Milkshake
Add all the ingredients, except for the ice, into a high-powered blender.

Blend until well combined.

Add in the (optional) ice and continue blending until cool and frosty.

Serve immediately. You can top with some whip cream and/or a drizzle of the salted caramel sauce.
☑️ Storage Instruction
This milkshake is best enjoyed fresh and does not store well once blended. But, if you have have leftovers you’d like to save for later you can pour them into a freezer safe container and freeze. Then let them sit at room temperature for several minutes before reblending.

💡 Tips
- Start blending without ice so the caramel sauce fully mixes into the base.
- Add almond milk slowly to control the thickness of the milkshake.
- Taste after blending and adjust sweetness if needed before serving.
- Use a high powered blender for the smoothest texture.
- Chill your glasses in the freezer for a few minutes before serving.
- Serve immediately for the best frosty texture.

🔄 Substitutions and Additions
Heavy cream: Blend in a tablespoon of heavy cream for an even richer milkshake.
Cocoa powder: Add a small amount of cocoa powder for a caramel chocolate twist.
Coconut milk: Use unsweetened coconut milk for a slightly different flavor profile.
Whipped topping: Top with sugar free whipped cream for a classic milkshake look.
Chocolate Sauce: If you’re a chocolate lover, you can drizzle a little of this homemade Chocolate Sauce over the top.

Why is My Milkshake Too Thin?
Your ice cream may not have been frozen solid or too much almond milk was added. You can add a few more ice cubes or a little more ice cream and blend again.
Can I Make This Milkshake Without a High Powered Blender?
Yes, but let the ice cream soften slightly first so it blends smoothly.
Can I Skip the Added Sweetener?
Yes, especially if your ice cream and caramel sauce are already sweet enough.
Can You Drink Milkshakes on Keto?
You can when you make them with all low carb and sugar free ingredients like this salted caramel milkshake recipe.
